The Facts

There are two types of the virus.

Type 1 herpes (HSV-1) - Cold sores
Type 2 herpes (HSV-2) - often associated with genital herpes

    Transmission

  • The virus is spread by skin-to-skin contact. Cold sores on the mouth are a potential source of genital infection during mouth-to-genital contact (oral sex).

  • Because recurrent herpes may cause few symptoms or pass unnoticed, it is possible to pass on the virus even when there are no visible blisters or sores.

  • Once you have a herpes infection, you probably cannot be infected again by the same strain of herpes virus.

    What are the symptoms of genital herpes?

    Although transmission to infants is rare (less than one per 2,000 births), genital herpes causes death or neurological damage in half of untreated newborns who become infected at birth.

    The first episode of infection, called the primary outbreak, is the most severe.

    Although herpes vaccine research is being conducted, no vaccine is currently available.

    Good rest, exercise and fresh fruit and vegetable juices will help strengthen the immune system against herpes.

    Conditions confused with Herpes
    Canker sores (aphthous ulcers): Occur inside mouth, gray with a distinct edge,usually heal within 10 to 14 days without treatment
    Bacterial or yeast infections: do not usually produce blistering
    Impetigo: A highly infectious bacterial disease, more common among children producing crusty blisters
    Syphilis: A bacterial infection, causes chancres. Syphilis does not usually produce a cluster of blisters, responds well to antibiotics.
    Molluscum contagious (molluscum): Like herpes, molluscum is a viral infection of the skin. Molluscum produces lesions that are raised, with a central dimple.

    Research from Medline Plus Health Information indicates that 90% of the population has been exposed to HSV-1 (herpes simplex virus) while 25% of the population between the ages of 25-45 have been exposed to HSV-2 (genital herpes). Both cause a lifelong infection.

    Herpes infections most commonly occur in the mouth, but can also occur in the genital areas and around the eyes. Some common symptoms are swelling, redness, clusters of blisters, fever, headache, fatigue, pain, and blurred vision. These symptoms are often the worst during the first infection.

    There are two types of the virus. Type 1 herpes (HSV-1) is carried by approx 90 per cent of the population and is the culprit behind recurrent cold sores. Most people pick it up in the first few years of life, usually in the form of a loving kiss from a parent or sibling.

    HSV invades the cells of the epidermis, the outer layer of the skin, causing fluid-filled blisters to appear. The virus travels from the epidermis along the nerve paths to the roots of the nerves where it becomes inactive.

    A weakening of the body's defences, due to a severe cold for example, can lead to a reactivation of the virus and a return of the blisters.

    HSV-2 affects five to 20 per cent of the population (including many individuals who are also infected with HSV-1). It is more often associated with genital herpes, although either virus can cause a similar disease at both anatomical sites.

    Genital herpes is one of the commonest sexually transmitted infections, and the prevalence is increasing. The number of cases caused by HSV-1 has grown due to changing sexual practices, including oral sex.

    Herpes can be spread by the following situations:

  • Kissing, touching and caressing infected areas
  • Sexual contact with the infected area (vaginal, oral, or anal sex)
  • Skin-to-skin contact with the infected area can transmit HSV-1 and HSV-2
  • Kissing someone if you have a cold sore can transfer the virus and the person you kiss can then contract herpes in the area kissed
  • The virus can be transmitted to your partner if you have active genital herpes and have vaginal or anal intercourse
  • If you have a cold sore and put your mouth on a partner's genitals (oral sex), the partner can be infected with genital herpes. Consequently, oral sex should be avoided if one partner has a facial herpes attack.
  • people who experience an episode of herpes, either facial or genital, should consider themselves infectious from the start of the episode up until the healing of the last ulcer
  • Occasionally, one partner in a long-term relationship may develop symptoms of herpes for the first time. Often this is due to one or both of the partners being carriers of HSV and not knowing it
  • From mother to baby during pregnancy or at birth

    The spreading of genital herpes through objects, such as soap, towels, clothing, bed sheets, toilet seats, and spa surfaces is highly unlikely because the genital herpes virus cannot live very long outside of the body. Herpes is not spread through vaginal fluids, blood or semen, or  through the air like a flu virus. Herpes is generally spread by direct contact.

    What can be done to prevent cold sores?

  • Avoid direct contact with the sores. These can appear on every
  • part of the body, including the fingers and genitals.
  • Wash your hands after touching the lips.
  • Avoid picking at the sores as this can spread the virus to other parts of the body or result in a bacterial infection of the sores.
  • The body's defences can be strengthened by a healthy lifestyle. Eat a varied diet, exercise regularly and get enough sleep.
    The factors which trigger outbreaks differ from person to person.
    Menstruation, fever and exposure to sunlight can cause a reactivation in some people. In others, there is no demonstrable cause. Using a sun block may help to prevent a reactivation.


    Simple treatments for the relief of discomfort and pain of genital sores.
     

  • Salt Baths - used to wash the genital area, can clean, soothe and dry the sores. Use 1 teaspoon of salt in 600 ml of water or a handful in a shallow bath.
     

  • Pain Relievers - include simple analgesics (such as aspirin and paracetamol), ice (which can be soothing if applied directly to the sores) and creams with an anaesthetic component. Creams, however, can slow down drying and should therefore be used sparingly and only for pain relief. In addition, topical creams have limited value on moist mucosal surfaces, particularly in women.
